In order to best serve patients, Board Certified Orthopedic Surgeon Jonathan Glashow, MD is an out of network provider. Still, most insurance companies, when the patient has a policy that allows them to go out of their network, will reimburse patients for most of the costs.
Please note: A doctor’s referral is NOT necessary from a General Practitioner to see our sports medicine or orthopedic surgery specialists for a consultation, 2nd opinion, or for treatment at our orthopedic practice – including surgery.
We will be happy to contact your health insurance on your behalf and assist you in filling out and filling all the paperwork.If you have healthcare insurance, most health care benefit plans provide “out-of-network” medical and surgical benefits for there members. With out-of-network benefits, members are entitled to payment for covered expenses if they use doctors and other health care professionals outside of the network.

YOU ARE RESPONSIBLE FOR ANY DEDUCTIBLE OR CO-PAYMENTS THAT YOUR INSURANCE COMPANY DOES NOT PAY. Dr Glashow’s Fees fall within most insurance carriers guidelines for the Usual & Customary range. Patients who are having multiple procedures with Dr. Glashow will be responsible for the PRIMARY procedure in full. Any additional procedures are payable at 50% of the surgical fee. The adjustments will be made in accordance with your INSURANCE GUIDELINES
If you have out of network benefits your insurance will provide payment for your orthopedic surgery. Usually they will amounts that are the lower of either:
- The out-of-network provider’s actual charge billed to the member, or
- The reasonable and customary amount,” “the usual, customary, and reasonable amount,” “the prevailing rate,” or other similar terms that base payment on what other healthcare professionals in a geographic area charge for their services.
