Forms and Privacy Practices: Below are forms which patients may fill out before coming to visit us, as well as our privacy practices.

Privacy Practices: How medical information may be disclosed and how you can access this information.

Vincent can assist you with:

• Making an appointment
• Messages for Dr. Glashow and all staff
• Helping patients fill out their initial evaluation forms
• Arrange for your MRI
• Collecting medical insurance information from patients
• Assist with prescriptions and pharmacies
• Letters of medical necessity , etc
• Letters to freeze gym memberships
• Contact information/ phone numbers for: Referring Doctors, Physical Therapist, Sports LabFitness Trainers

Pre-Operative Testing: For patients under 16 years old, you need to be seen by a Pediatrician within 30 days of your surgery.

For patients over 50 years old, you need:

  •  To be seen by an Internist for clearance with a History & Physical.
  •  Have an EKG within 30 days of your surgery.
  • Blood work within 30 day-SMA 20 including liver function, CBC, PTT.
  • Urinalysis
  • MRI

For Patients over 60 years old you will also need a chest x-ray.

If you are being treated for a chronic illness, you will need to be seen by your treating Physician to obtain surgical clearance regardless of your age.

Medications: Special instructions about your medication(s) in the days before your surgery may be given to you by Dr. Glashow or by the anesthesiologist.

10 DAYS PRIOR TO YOUR SURGERY You must STOP taking ASPIRIN, IBUPROFEN containing compounds – unless otherwise instructed by your physician. Also STOP taking Vitamin E or any supplemental vitamins which contain E (this includes multi-vitamins) for10 days prior to surgery. Please ask your Personal Physician regarding the stoppage of any prescription medicine. You may take Tylenol as needed.

Stop Smoking: Along with long term health problems, research shows that smoking can increase the chances of complications of surgery and anesthesia. There is no smoking allowed inside of hospital facilities. For help quitting including counseling, support, nicotine replacement, etc. speak with your doctor.

Financial Planning :Questions related to charges for your surgery should be addressed early.

You will be receiving separate bills from the following: Dr.Glashow, Anesthesiology, Pathology, Gotham Surgical & Brace (Cryocuff /Gameready), and Mount Sinai Hospital.

Gotham Surgical & Brace will send an ice machine, Cryrocuff or Gameready to the hospital for you to take home. If you prefer you may have this shipped to your home. Please call Yesica at 212.297.0362. She will give you instructions on its use. Please use your ice machine until otherwise directed.

Reminder: Please have a large supply of ice at home after your surgery.
